What are the requirements to be submitted for membership application?

  • MEMBERSHIP DOCUMENTARY REQUIREMENTS

STEP 1 :

Documents needed:

  1. Membership Application
  2. Signature Card
  3. 1×1 ID Picture
  4. Photocopy of valid ID (TIN,SSS, and other government issued ID)

STEP 2 :

Send the scanned copy of your documents via

STEP 3 :

Wait for Cooperative Confirmation & Instruction through email

STEP 4 :

Payments and Deposits through Bank or E-wallets

Share Capital to put-up if savings only:  

               Share Capital                    2,000.00

               Regular Savings                   500.00

               Membership Fee                 100.00

               MAP                                     1,000.00 (Mortuary Assistance Program)

                                             TOTAL  3,600.00

Share Capital to put-up if you want to loan:

               Share Capital                    3,750.00

               Regular Savings                   500.00

               Membership Fee                 100.00

               MAP                                     1,000.00 (Mortuary Assistance Program)

                                             TOTAL  5,350.00

STEP 5 :

Scan the deposit slips and send through email as proof of deposit.
